REGISTRATION CHECKLIST

Use this checklist to review the information that you need to register for the CPACE.

1. Review your registration options.

2. Prepare your payment.
Payment must be made by credit, debit, or check card (VISA or MasterCard only) as described in "Test Fees and Payment Policies."
3. Gather the personal information you need to register.
When you register for the CPACE, you will be asked to provide the following information:
  • Name (use your legal name that appears on your government-issued identification)
  • Address
  • Daytime telephone number
  • U.S. Social Security number (required for registration and to access your scores online)
  • Date of birth (required to access your scores online)
  • Email address
  • Ethnicity (optional)
  • Gender (optional)
The accuracy of your Social Security number and date of birth is an essential part of the registration and score-reporting process. It will ensure the accurate matching of your CPACE records no matter how many times you take each of the components.
4. Review the background questions.
When you register for the CPACE, you will be asked to answer some background questions. Refer to "Background Questions" to prepare answers to the questions you will be asked during registration.
5. Review the Rules of Test Participation.
Before you register, you must read and agree to abide by the policies stated in "Compliance with Testing Rules," including the Rules of Test Participation, and all procedures and policies contained in the 2012–2013 CPACE Registration Bulletin.
